Important document that should be completed by all players at the beginning of each season and included in every trainers kit. 

Goaltender Registration Credit Application 

A credit towards the following season's registration fee will be issued to all dedicated house league goaltenders (excluding U7), provided they:

• have paid the current season registration fees in full by the registration deadline
• have completed the entire season as a goaltender
• attended CGHA sponsored goalie clinics (if offered) 

Registration Refund Request Form

To request a refund on regular season and clinic registrations. Conditions apply. 

Certification Reimbursement Claim Form

To request reimbursement of courses and clinics required by OWHA. Conditions apply. 

Fundraising Request Form

In accordance with the CGHA Fundraising Policy, teams wishing to fundraise must submit a request to do so (see final page of policy).
